2021-09-022024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/651902本研究欲探討職業相關的抗生素抗藥性的全球問題。在過去的幾十年裡,抗生素抗藥性成為一個日益受到關注的議題,因為它危及細菌感染的治療。抗生素抗藥性是個複雜且多因素的議題,過去的研究顯示特定職業有更高的抗藥性細菌感染風險。這有部分是由於在這些職業的環境中更有機會暴露於抗生素,但在全球仍缺乏全面的概述、風險比較和指引。台灣長期收集全面且高品質的資料,因此有更好的機會了解抗生素抗藥性與職業之間的關係。除了可以增進國家利益,台灣的見解也將有助於全球對抗抗生素抗藥性的對策。本研究計劃擬對抗生素抗藥性與職業之間的關係進行廣泛且統括式的研究,首先對科學文獻、管理策略、政策和管理資料庫進行系統性的回顧,以全面性的了解當前的知識和知識差距。其次,我們建議結合勞保局和全民健康保險資料,建立回顧性的數據世代,並以此世代進行量性的統計分析,以取得目前台灣特定職業在抗生素抗藥性感染方面的詳細資訊和風險評估。最後,我們建議將本研究的結果撰寫成雙語手冊,以提供特定職業未來管理抗生素抗藥性的指引。此外,在整個研究執行期間,將透過辦理專家會議和實地訪視諮詢其他專家和利益相關者,以確保透過跨領域合作和透明化的方式來解決這一個複雜的全球議題。 This research proposal focuses on the global issue of antimicrobial resistance (AMR) in relation to occupation. Over the past few decades AMR became a topic of growing concern, as it is endangering medical treatment for bacterial infections. AMR is a complex and multifactorial issue, and literature has indicated that certain occupations are more at risk for developing resistant bacterial infections. This is partially due to higher exposure to antibiotics in these occupational settings, but comprehensive overviews, risk comparisons and guidelines are missing globally. Due to its comprehensive and high-quality data collection, Taiwan has an excellent opportunity to obtain a better understanding of the relation between antibiotic resistance and occupation. Besides national gains, Taiwanese insights will also contribute to the global actions against antibiotic resistance. This research plan is proposing to conduct a broad and inclusive study on the relation between antibiotic resistance and occupations, firstly by conducting a systematic review on scientific literature, management strategies, policies and management databases, a comprehensive understanding of the current knowledge and knowledge gaps will be achieved. Secondly, we propose to establish a retrospective data cohort by combining Bureau of Labor Insurance datasets and National Health Insurance datasets and appropriate statistical analysis will be conducted on the dataset to obtain current detailed information and risk assessments for specific occupations in Taiwan regarding antibiotic resistant infections. Lastly, this study proposes to communicate all its findings by the establishment of a bilingual handbook per occupation, to better manage the risk of antibiotic resistance in the near future.
