2011-08-012024-05-13https://scholars.lib.ntu.edu.tw/handle/123456789/645464摘要:「職業傷病」包括因工作引起的急性災害事故與慢性疾病。職業傷病的監測通報與認定,乃是國家職業健康政策中相當重要的一環。工業先進國家皆對職業傷病的發生狀況與其原因,建立監測與統計分析機制,然而,各國職業傷病低估的狀況卻十分普遍,尤其是因果關係不易釐清的慢性疾病,一直是各國重要的職業健康治理議題。職業傷病的監測與通報機制之核心爭議,在於傷病問題「職業相關性」之認定;此認定過程不僅牽涉到職業醫學與流行病學的因果估算,更牽涉到勞雇雙方的責任歸屬,以及社會大眾對職業風險責任認知程度。本計畫旨在深入探討我國現行的職業傷病監測通報與認定機制,瞭解制度設計與實務運作現況,並將透過日、韓制度之比較,以釐清我國制度問題與改革方向,亦將就社會參與機制作初步探討。本研究亦將以醫療照顧服務業工作者作為研究族群,以瞭解現有職業傷病監測通報機制之適用性,理由包括:醫護照顧工作者的職業傷病風險值得關注;醫護工作者相較於其他職業族群更瞭解職業健康風險;在職業傷病監測通報制度中亦屬第一線人員;此外,傳染病防治政策中另訂工作者因工作感染傳染病之補償辦法,與既有的職災補償制度之關連,亦將進一步探究。(486 字)<br> Abstract: The schemes of monitoring, reporting and recognizing occupational injuries and illnessesform the cornerstone of the occupational health systems. In all developed countries,surveillance systems have long been established to investigate the occurrences ofoccupational health problems as well as their causes. Nevertheless, under-reporting is stillcommon, especially for illnesses with long latency period and with multiple causes. Theevaluation of an event’s ‘work-relatedness’ has been the center of controversy, which involvenot only scientific evidences with regards to its causality, but also perception and attitudetoward the responsibility for its occurrence.This project aims to explore the current occupational health surveillance systems inTaiwan, both in design and in practice. In order to better comprehend the problems of thesystems in Taiwan, we also plan to conduct comparative study with the systems of Japan andKorea, with special attention toward the mechanisms for the enhancement of socialparticipations. Furthermore, surveillance systems for workers in the health care sector willbe studies, based on the following considerations: first, occupational health risks amongworkers of this growing sector deserve attention; second, health care workers play anessential role in the surveillance system for workers from all other sectors; and lastly,co-existing compensation systems peculiar for health care workers inflicted work-relatedinfectious diseases seem to complicate the generic systems and therefore deserve furtherinvestigation.
