鄭素芳2006-07-262018-07-082006-07-262018-07-082003http://ntur.lib.ntu.edu.tw//handle/246246/25628嬰兒出生一年內所出現的跨步動作可能是獨立行走的前遣動作，過去有不少 研究曾經藉由探討足月兒跨步動作的成熟過程以幫助瞭解嬰孩行走發展，然而類 似的資料在早產兒方面則闕如。因此本研究採兩年之前瞻性追蹤設計，目的有 二：一、藉由跑步機支撐跨步的運動學測試探討早產兒與足月兒從矯正年齡７個 月到行走達成之下肢動作發展過程，二、探討可能影響早產兒與足月兒行走達成 年齡之因子。本研究共收集台大醫院出生的足月兒（20 名）、低危險早產兒（14 名）、以及高危險早產兒（14 名），三組嬰孩的性別及父母社經狀況相配合。受 試嬰兒達矯正年齡7 個月時接受第一次檢查，之後每兩個月做檢查直到行走達成 或18 個月大為止。每次追蹤所檢查之內容包括：跑步機支撐跨步、粗動作發展、 體格、身體組成及認知理解能力。本研究的結果將幫助吾人瞭解早產兒與足月兒 的行走發展過程，所找出與行走發展遲緩相關的早期神經動作問題與可能影響之 因素將有助於早療專業人員提供早產兒早期、有效的治療計畫。Supported stepping movements have been proposed as the precursors to independent walking and their maturational process has been extensively studied in term infants to help understand walking development. However, the information concerning walking development on preterm infants is limited. Therefore, the purposes of this prospective, two-year follow-up study were twofold. First, we examined the development of leg movements in preterm infants and term infants when supported stepping on a treadmill from 7 months corrected age to walking attainment. Second, we examined factors that may influence the age of walking attainment of preterm infants and term infants. This study included term infants (n= 20), low-risk preterm infants (n= 14), and high-risk preterm infants (n= 14) who were born at the National Taiwan University Hospital. The groups were matched for gender and parental socio-economic status. Infants were scheduled for the first examination at 7 months corrected age and were followed up every two months until walking attainment or 18 months corrected age. At each visit, infants were examined for supported stepping movements on the treadmill, gross motor development, anthropometry, body composition and cognition. The results of this study will help understand the walking development of preterm infants and normal term infants.
