How to search and access intelligently on content based image database become an important focus in the field of multimedia research. In this study, a preliminary system of grid-distributed, contextual and semantic based, intelligent information access framework for medical images database was designed to integrate with radiology reports and clinical department information. A prototyping was established with a model of intelligent diagnostic and therapeutic decision support for brain degenerative diseases among radiologist and neurologist. Dementia was the first disease selected for implementation.
