2016-08-012024-05-17https://scholars.lib.ntu.edu.tw/handle/123456789/695459摘要：隨著醫療科技的進步，超過九成的先天性心臟病人可存活至成人期，但許多病人仍受到疾病合併症的威脅，且隨著年齡的增長可能有較容易罹患慢性病的風險，採行健康行為被認為有助於此群病人維持健康，然而實證研究仍不足。本研究之目的在比較先天性心臟病成人與一般成人之健康狀況，並探討與一般成人相比，健康行為與先天性心臟病人之健康狀況的關係。本研究為三年計畫，第一年以縱貫式、世代追蹤研究設計，使用全民健康保險研究資料庫，除了探討先天性心臟人的疾病合併症、慢性疾病、女性病人的孕期合併症的盛行率，更分析隨著病人年齡增長，各種疾病盛行率的變化趨勢，並與一般成人進行比較。第二及第三年之計畫將以縱貫式、病例對照研究設計，以問卷調查、生理測量及病歷查詢之方式，收集受試者之健康行為、心臟病相關合併症、慢性疾病罹病資料與各項生理指標，以探討健康行為與先天性心臟病人健康狀況間的關係，並與一般成人進行比較，以了解健康行為對先天性心臟病人之相對影響力。藉由本研究之分析，將得到先天性心臟病人之疾病合併症與慢性病的盛行率及其變化趨勢，並了解健康行為對先天性心臟病人之相對成效，結果可做為發展先天性心臟病人長期照顧指引之參考。<br> Abstract: With advances in medical and surgical therapies, 90% of patients born with congenital heart disease (CHD)are expected to live to adulthood. However, cardiac complications are an ongoing threat for many patientswith CHD. Compared with the general population, such patients are more likely to acquire chronic diseasesas they age. Empirical research on whether patients with CHD can benefit from adopting health behaviors isscant. Therefore, this 3-year proposal is aimed at comparing the health statuses of adults with and withoutCHD by examining the association between health behaviors and health outcomes. In the first year, we willconduct a longitudinal cohort study by using data from the National Health Insurance Research Database toanalyze and compare the prevalence of cardiac complications, acquired chronic diseases, and obstetriccomplications in adults with and without CHD. Furthermore, the trends in disease prevalence over the pastdecade will be explored. In the second and third years, a longitudinal case-control study will be conducted.Information on health behaviors, cardiac complications, and physical parameters will be collected usingself-report questionnaires, physical measurements, and medical charts.
