Abstract
An anal fistula without an external orifice sometimes extends upward from the intersphincteric layer to the pelvic cavity. Early presentations may be uncommon, and misidentified as manifestations of other diseases. Such a case simulating benign prostate hypertrophy is presented. The patient visited this Hospital because of the hesitancy and intermittence of his urinary stream, but perianal reddness and tenderness had developed later. After diagnosis as a pararectal abscess, he received three operations and recovered without sequelae. Pararectal abscess should be considered as a possible diagnosis for pelvic mass of unknown origin. When it is suspected, Computer tomography and endoanal ultrasonography are helpful to secure accurate diagnosis and treatment.
