Methods: Six consecutive patients suspected on the basis of clinical appearance and abnormal chest radiographie findings of having PAVMs were included in the study. Real-time gray-scale imaging was performed followed by color Doppler imaging and amplitude US angiography in a random order. All patients were later proved with angiography to have PAVMs. Results: The US study was successfully performed in all six patients. Eight lesions were detected. The real-time gray-scale image of PAVMs revealed well-defined, hypoechoic subpleural nodules with strong posterior acoustic enhancement. Color Doppler US of PAVMs showed turbulent flow, which manifested as an area of intense color with high and mixed velocities (reticulated or mosaic-like pattern). Anatomic continuity was demonstrated in some PAVMs. Amplitude US angiography can delineate a tangled vascular structure with a clear vessel wall and anatomic continuity as well as can conventional angiography. Spectral wave analysis showed a relatively low impedance flow with high peak systolic velocity (mean, 44.4 cm/sec) and relatively high diastolic velocity (mean, 19.3 cm/sec). The mean pulsatility index and resistive index were 1.80 and 0.49, respectively. In two patients who received embolotherapy, the color Doppler US scan obtained after the procedure showed that the previous focal areas of color flow signals disappeared or diminished in size. This was compatible with the decrease in or absence of blood flow demonstrated at angiography after embolotherapy. Conclusions: Combined color Doppler US and amplitude US angiography are useful, noninvasive techniques for diagnosing PAVMs, and they provide an alternative approach to angiography in evaluating the effi-rarv of pmholotheranv. |
