Abstract
Taiwan's healthcare system operates under a universal, single-payer National Health Insurance (NHI) that covers every citizen.All medical services are paid under government-set fees, ensuring access and cost control but limiting flexibility for adopting new technologies.Recently, self-pay services have emerged, allowing patients to access advanced innovations-such as artificial intelligence (AI)assisted imaging-before they are incorporated into the NHI schedule.Meanwhile, Taiwan-often called the "Silicon Island" for its global semiconductor and hardware leadership-has a strong base for digital transformation.Both government and private sectors actively promote AI development and clinical use, fostering rapid translation into practice.Within this context, AI integration in radiology has accelerated, supported by international vendors and domestic start-ups.This editorial outlines Taiwan's current experience with AI access and reimbursement and discusses how a physician-centered, human-in-the-loop model can support sustainable implementation.
