Abstract
We report a rare case of double outlet right ventricle with transposition of the great arteries, subpulmonary ventricular septal defect, pulmonary valve stenosis, and superior-inferior ventricles successfully managed by the Nikaidoh procedure. Preoperative 3-dimensional simulation clarified the complex spatial anatomy and confirmed feasibility without inflow compromise. Surgical results mirrored the simulation, with smooth recovery and excellent 4-year outcome. The Nikaidoh procedure is feasible in cases with superior-inferior ventricular arrangement and pulmonary stenosis, and 3-dimensional simulation can facilitate precise surgical planning, potentially improving outcomes in similarly complex congenital heart defects.
